I know many sources have been listed elsewhere on the site, but for the purposes of the GK manual I would like to collect them here. 

Without meaning to sound like we're at school, it would be helpful to have as much information as possible to track these down.  Please list (if you can):
1) Author
2) Title
3) Format (DVD, book, website, etc)
4) Year of publication if known
5) Publisher (if applicable)
6) Source (where you got the resource from, including URL if possible)
7) What about it makes it useful
